Please complete this form before purchasing your place on The Reset: 6-Week Transformation Challenge.

By signing this form, I confirm that:

• I am 18 years of age or older.

• The information I have provided is true and accurate.

• To the best of my knowledge, I am medically fit to participate in this Challenge or have sought appropriate medical advice before taking part.

• I understand that exercise involves inherent risks and that I participate voluntarily and at my own risk. I will stop exercising immediately and seek medical advice if I experience pain, dizziness, chest pain, unusual shortness of breath or any other concerning symptoms.

• I understand that the training programme, nutrition guidance and educational resources are provided for general health and wellbeing purposes only and do not replace medical advice.

• I understand that Abby Leyland is acting as a Health & Wellness Coach and is not providing medical, physiotherapy or dietetic services.

• I acknowledge that individual results vary and cannot be guaranteed.

• To the fullest extent permitted by law, I release Waking Life Wellness and Abby Leyland from liability for injury, illness, loss or damage arising from my participation, except where liability cannot legally be excluded.

• I agree to behave respectfully within the Challenge community and understand that abusive or disruptive behaviour will result in removal from the Challenge without refund.

• I understand that the £60 Challenge fee is non-refundable.

• I understand that all programmes, guides, trackers and resources remain the intellectual property of Waking Life Wellness and are provided for my personal use only. They must not be copied, reproduced, shared, distributed or used for commercial purposes without prior written permission.

• I understand that my personal information will be processed solely for administering the Challenge in accordance with applicable UK data protection legislation.

• I understand that, to enter the Prize Draw, I must grant Waking Life Wellness permission to use my before-and-after photographs, before-and-after videos, first name, and transformation story for promotional and marketing purposes, including sharing across relevant marketing channels.

• I have read, understood and agree to the Terms & Conditions and the Before & After Photo and Video Verification Guidelines.
